Services Provided
We specialise in conducting medical assessments for:
- D4 Medical Assessments: Required for Heavy Goods Vehicle (HGV), Passenger Carrying Vehicle (PCV), and bus drivers as mandated by the Driver and Vehicle Licensing Agency (DVLA).
- PCO Medical Assessments: Necessary for private hire and taxi drivers as required by Transport for London (TfL).
Booking and Payment
Appointments: Appointments can be scheduled through our website or by contacting us directly. Please ensure all information provided during booking is accurate.
Payment: Full payment is required at the time of booking. Payments are processed securely through our website.
Cancellations and Refunds
Cancellations by You: If you need to cancel or reschedule your appointment, please notify us at least 24 hours in advance. Cancellations made within this timeframe may be eligible for a refund or rescheduling.
Cancellations by Us: We reserve the right to cancel or reschedule appointments due to unforeseen circumstances. In such cases, we will offer an alternative appointment or a full refund.
Client Responsibilities
Documentation: Clients must bring their own D4 or PCO forms to the appointment. For PCO Medical Assessments, a medical summary from your GP is required. Failure to provide these documents may result in the inability to conduct the assessment.
Accuracy of Information: You are responsible for providing accurate and complete information during the assessment. Any omissions or inaccuracies may affect the outcome of your medical certification.
Medical Assessment Outcome
No Guarantee: The outcome of the medical assessment is based on the results obtained during the evaluation. We do not guarantee that every client will pass the medical examination.
Reporting: Assessment results will be documented on the appropriate forms. It is your responsibility to submit these forms to the relevant authorities (e.g., DVLA, TfL).
Confidentiality and Data Protection
Privacy: We are committed to protecting your privacy and personal data. All information collected during the assessment process will be handled in accordance with our Privacy Policy.
Data Sharing: Your personal data may be shared with relevant authorities, such as the DVLA or local licensing bodies, as required by law.
Limitation of Liability
Scope: Our liability is limited to the services provided during the medical assessment. We are not liable for any indirect, incidental, or consequential damages arising from your use of our services.
